What Is a Care Plan?
Your care plan is the playbook from which your team will provide care. It contains an assessment of your health, your diagnoses, and your expectations for treatment as well as your care team’s expected outcomes. Based on this, your team will recommend medical interventions (treatment, surgery, medications, etc.) along with the reasons for recommending these interventions. A care plan covers your medical needs. Our customized care plans take into account your emotional and personal needs as well as your medical needs.
Rather than leaving every individual nurse who cares for you to come up with their own ideas for treatment, a care plan ensures that everybody who comes in contact with you has the most up-to-date information and can provide a consistently high level of care.
Customizing the Care Plan
No two lives are exactly the same. We each have unique experiences, histories, living arrangements, expectations, and needs. When a care plan is personalized, your care team takes your own desires and expectations into account. We speak with caregivers, family members, and other people who will be involved with your care or recovery. This gives us a better idea of who you are, what kind of support you have, and what “getting better” looks like to you.
One patient, for example, might be happy to manage pain with medication. Another might not be able to take certain medications due to allergies or interactions, or because they have a job that requires them to be fully alert for safety reasons.
